What you need

  • Responsible-lawyer direction on the matter purpose and permitted request pathway
  • Verified client or patient identity information
  • Available provider names, facilities, dates of service, referrals, bills, insurance statements, and matter records
  • The firm's approved secure location for protected health information
  • A request tracker or equivalent case-management record
