What you need
- A reviewed request plan from the provider, authority, and scope workflow
- A valid client authorization and any required representative documentation
- The provider's current official requester-specific instructions
- An approved secure transmission, delivery, and storage location
- A request tracker with one line for each holder and separately routed record category

- 1Open the approved request plan and confirm that the holder, requester pathway, authority, scope, special-record flags, secure destination, owner, and reviewer are resolved. Stop if the plan is incomplete or the task has changed.
- 2Reopen the provider's current official instructions on the submission date. Verify the facility or practice, department, requester category, portal or address, required form, required attachments, and any provider-stated fee or timing information; record the date checked.
